Analysis warns of CV risks linked to albuterol

A decade-long review of clinical data has raised fresh concerns about the cardiovascular risks associated with albuterol, a widely used asthma medication. The study, “A 10-Year Interval of Cardiovascular Effects of Albuterol in Asthma Management: Graphical Review,” was published in the journal, Current Research in Pharmacology and Drug Discovery.

Albuterol, also known as salbutamol, is a short-acting beta-agonist (SABA) commonly prescribed for rapid relief during asthma attacks. Although its bronchodilator effects make it a cornerstone of emergency asthma care, researchers warn that its impact on the heart may be underestimated.

Researchers analyzed literature across PubMed, Cochrane and Google Scholar between 2015 and 2025 to assess frequency, severity and risk factors associated with cardiovascular events, including tachycardia, arrhythmias, QT interval prolongation and hypotension, particularly in vulnerable populations.

According to the study’s authors, cardiovascular risk associated with the use of albuterol was linked to:

  • Certain groups: Children, elderly patients and individuals with pre-existing heart conditions face increased susceptibility to adverse effects.
  • Medication administration: Intravenous albuterol delivery showed significantly higher cardiovascular risks compared to inhaled forms.
  • Electrolyte imbalances: Continuous therapy in pediatric patients was linked to hypokalemia and hypotension.

Researchers also looked at the use of the alternative asthma inhaler, levalbuterol, which offered a safer cardiovascular profile but came with higher health care costs and longer hospital stays to treat an asthma attack.

The study’s authors urged clinicians to adopt personalized dosing strategies, conduct thorough cardiovascular risk assessments and consider safer alternatives when possible. The review also called for enhanced monitoring protocols, including regular ECG checks and potassium level assessments for high-risk patients. 

Looking ahead, researchers recommended additional large-scale randomized trials and pharmacogenomic studies to better understand genetic factors influencing drug safety. They also suggested leveraging artificial intelligence (AI)-driven pharmacovigilance systems to detect early warning signs of adverse effects.

Although albuterol remains an essential tool in asthma management, this review underscores the need for caution, tailored prescribing and ongoing observation to protect patient safety.
